The process is easy - simply complete the form below with all of the information requested and return to us. At the first of every month, we will send out the spa gift and an inspirational saying to the lucky recipient…that is all you need to do! Once a month, we will also send you an email to confirm that the gift has been sent. If you have any special requests, you can call us directly.

Once we receive your enrollment form, we will contact you to confirm the details and discuss payment options.  We take American Express, Visa, Mastercard and Discover.

We are happy to work with you on special requests or alternating the monthly gifts, based on availability. Cottage Breeze Day Spa & Boutique reserves the right to substitute any monthly gift with a similar one of equal or greater value.
